A forklift certificate template gives you a clear record for the operators you train. Employers, safety trainers, equipment dealers and staffing agencies hand one out when a person has finished the classroom part and passed the practical evaluation on the truck. Pick a forklift training certificate template or a forklift badge template here: the designs come as certificates (A4 and US Letter) and as badges.
A good certificate for forklift operators names the operator and the truck class, says what was covered, such as the pre-use inspection, load stability and pedestrian awareness, and shows the date, the standard and who signed. In the United States the standard is OSHA 29 CFR 1910.178, and operators must be re-evaluated at least every three years, so a valid-until date is useful.

FAQ
Questions, answered
The operator's name, the truck type they were trained on, the date of the practical evaluation, the standard followed and the signature of the trainer or employer. Many add the topics covered and a valid-until date.
Under OSHA, operators are evaluated at least once every three years, so most employers set the valid-until date three years after the evaluation. Retraining sooner is needed after an accident, a near miss or a change of truck.
The person who ran the practical evaluation, such as a qualified trainer or safety manager, and often the employer or site manager as well. OSHA wants the employer to certify that the training and evaluation took place.
No. In the United States there is no national forklift licence. The employer certifies that the operator was trained and evaluated, and the certificate is that record. Other countries may have their own licence rules.
Yes. Training is tied to the type of truck, such as a Class 1 counterbalance or a Class 2 reach truck, so naming the class shows what the operator may drive.
It is the stability triangle used in forklift courses. It joins the two front wheels and the pivot of the steer axle, and the truck stays upright while the centre of gravity stays inside it.
