A CME certificate template gives a continuing medical education provider a ready record for each learner: the physician's name, the activity, the date and the credits claimed. Medical schools, hospitals, specialty societies and education companies issue one after a conference, a grand rounds series, a journal-based module or an online course, and clinicians keep it for licence renewal, hospital privileges and certification audits.

A good continuing medical education certificate names the activity exactly, shows how many credits the learner claimed, and leaves room for the provider's own credit designation statement, which only the provider can write. The topics covered, the learning format, a signature from the course director and a credential ID that can be verified make it a complete record.

The designs come as certificates, on A4 and US Letter, and as badges that clinicians can add to a professional profile.

Wording

What to write on a continuing medical education (cme) certificate

  • “This certifies that the physician named above participated in the activity and claimed the credits shown.”
  • “Awarded for completing this continuing medical education activity and its learning assessment.”
  • “For participation in an educational activity on current evidence and clinical practice.”

Lines for conferences, grand rounds and online CME

  • “For attending the annual clinical update and completing the post-activity evaluation.”
  • “In recognition of participation in the grand rounds series, January to June.”
  • “For completing the online module and its self-assessment questions.”

FAQ

Questions, answered

The learner's name, the title of the activity, its date and format, the provider's name, the number of credits claimed and the provider's credit designation statement. Many providers add the topics covered, a signature from the course director and a credential ID so a licensing board or an employer can check the record.
