Design notes

The tall layout gives the forest plot more room: larger rows for the four topics covered, each with its credits on the right, and a wider burnt-sienna diamond for the credits claimed. Above it sit a large serif title, the physician's name and the activity in italic. Below it come a placeholder for the provider's credit designation statement, a short summary, and a two-by-two block with the learning format, duration, date of issue and credential ID. A signature with its role and a QR code close the page. It suits providers who want an academic, evidence-based look.

About

CME Certificate Template, Forest Plot Portrait

Continuing medical education certificates often end up in a portfolio, uploaded to a state board or a credentialing office, or printed for a CME folder, and a portrait page suits those files and binders. Medical schools, health systems, CME companies and professional associations issue one after each activity, so a clinician can show what was learned and how many credits were claimed when a licence or certification comes up for renewal.

Wording on the template“The physician named above participated in this continuing medical education activity and completed its learning assessment, bringing current evidence and updated guidelines into clinical practice.”

Subject: Your CME certificate is ready, name

Hi name,

Thank you for taking part in our continuing medical education activity and for completing the evaluation. Your certificate of credit is ready. It shows the activity, the date and the credits you claimed, with a credential ID that a licensing board or your employer can verify.

Open it to view the record, download a copy for your CME file or print it, and share the badge on your professional profile if you wish.

With thanks from the education team.

Hyperstack fills in each person’s name and link when it sends.

Post for the recipient to share

I completed a continuing medical education activity this week and claimed my credits for the current cycle. Keeping up with new guidelines and diagnostic advances is part of caring for patients well, and the sessions gave me ideas I am already using in clinic. Thanks to the faculty for a well-run program. #CME #MedicalEducation

Printing

Printing it well

Print at actual size

Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.

Use card, not paper

A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.

Frame it

A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.

Mind the edges

Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.

Certificate or badge?

Send both. They do different jobs.
