A CNA certificate template helps a training program give each graduate a clear record of the course: the hours of classroom and clinical training, the skills a nurse evaluator checked off and the date it all ended. Community colleges, career schools, high school health programs and nursing homes that train their own aides hand it out on the last clinical day, often at a small pinning or graduation.

It is a training record, not a licence. The graduate takes it to the state competency evaluation, and only the state can list them on the nurse aide registry with the certified nursing assistant title. So a good nursing assistant certificate names the training provider and the program, the hours, the skills practised and the date, and carries the program director's signature.

The designs here come as printable certificates in A4 and US Letter and as digital badges to share online.

Wording for the skills check-off

FAQ

Questions, answered

No. The certificate shows that the person finished a training program. To work as a CNA they must also pass the state competency evaluation, usually a written or oral test and a hands-on skills test, and be placed on the state nurse aide registry. Word the certificate as a record of training, and do not say that the person is certified.
