Design notes
The tall layout gives the name more room. The title splits over two lines, a four-column row holds the training hours, format, check-off result and issue date, and then the gait belt crosses the page with its nickel buckle at the centre. Under the belt, five skills sit in two columns with tick boxes, followed by two signatures with the QR code and credential ID between them. Its warm cotton paper and deep green ink suit a care setting.
About
Nursing Assistant Certificate Template, Gait Belt Portrait
Many nursing assistant graduates keep their training record in a folder for the state exam and for job applications at care homes, hospitals and home health agencies, and a portrait page fits that folder. Programs issue it at the end of the course, once the instructor has observed every required skill, from bed baths and feeding to taking a pulse and walking a resident with a gait belt. It records the training; the CNA title comes later, from the state, after the competency evaluation.
Wording on the template“Completed classroom instruction, skills-lab practice and supervised clinical hours in resident care, and performed every skill on the competency check-off to standard, observed and signed off by a registered nurse evaluator.”More wording ideas for nursing assistant (cna) training certificates

Subject: name, your nursing assistant training certificate is ready
Hi name,
Congratulations on finishing the Nursing Assistant Training Program. You put in the classroom hours, practised every skill in the lab and cared for residents during your clinical shifts, and your evaluator signed off each skill on the check-off.
Your certificate of completion is ready at the link below. You can download it, print a copy for your exam papers and share it with employers. Keep it with your documents for the state competency exam.
We are proud of you and wish you well in the work ahead.

Post for the recipient to share
I have finished my nursing assistant training: 120 hours of classroom lessons, skills lab and clinical shifts in long-term care, with every skill checked off by our nurse evaluator. Thank you to my instructors and to the residents who taught me so much. Next step: the state competency exam. #CNA #NursingAssistant #Healthcare

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
