An OJT certificate template helps a host company record that a trainee has finished on-the-job training: supervised, real work inside the business until the required hours are complete. In the Philippines, college students render OJT hours as part of their degree and senior high students complete work immersion; in workplaces everywhere, employers use OJT to train new hires, apprentices and staff moving into a new role.
A good on-the-job training certificate names the trainee and the host company, states the hours rendered and the training period, lists the department and the main tasks or competencies, and carries the signatures of the training supervisor and an HR officer or school coordinator. The designs here come as certificates in A4 and US Letter and as digital badges that trainees can add to a profile.

FAQ
Questions, answered
Usually not. A certificate of employment confirms that a person held a job with the company, while an OJT certificate records a training placement: supervised work, done to learn, for a set number of hours. Student trainees receive the OJT certificate.
Yes. Schools usually check the hours against the total their program requires, so write the exact number of hours rendered and the start and end dates. Use the figure from the approved time records or daily time record, not an estimate.
Ideally both. The direct supervisor at the host company saw the work and confirms the hours, and an HR officer or department head signs for the company. For school placements, the OJT coordinator may countersign or keep a copy for the student's file.
Once the trainee has rendered the required hours and the supervisor has completed the final evaluation. Many schools need the certificate before they can grade the practicum, so issue it within a few days of the last day.
They record the same kind of experience. In the Philippines, work immersion is the term for senior high school placements, while OJT or practicum is used for college students and for employer training. Use the term the school or program uses.
