A dance certificate template gives a dance studio, a school dance program or a community arts group a ready page for the moments that mark a dancer's progress: a first term of ballet, a graded exam in tap or jazz, a level finished in contemporary or hip hop, a season with the dance team. Teachers usually hand them out at the end-of-year recital, the last class of term or the day the exam results come in.
A good certificate for dance names the student and the style or level, says what they worked on (technique and alignment, musicality, remembering a routine, performing with confidence) and carries a date and the teacher's signature. For younger children a line about effort or progress often means more than a grade.

It is the most common moment, because families are in the audience and every dancer is on stage. Many studios also give certificates at the last class of term, when graded exam results arrive, when a dancer moves up a level and at the end of a competition season, and add a few special awards at the recital.
For young children, effort and progress usually matter more than a grade. Name the class (for example Pre-Primary Ballet or Junior Tap), add one line on what they did well, such as listening, remembering the steps or performing on stage, then the date and the teacher's signature.
Often both. The class teacher signs for the lessons and the progress, and the principal or artistic director adds a second signature, which makes a level or exam certificate feel more official.
Put the style and level in the program line, for example Contemporary, Level 3, and list a few of the things covered, such as technique, choreography, musicality and performance. Use the same wording each year so dancers and parents can see the steps they have climbed.
They are stage numbers. Many stages have them marked along the front edge, counting out from 0 at centre in both directions, so dancers can find the same spot in every rehearsal and show. The small T-shaped mark under the name is spike tape, which marks a dancer's exact place on the floor.
