Design notes

The tall page puts the record first: the program, the name in a calm serif, a one-line statement and five skills chips. Below them the provider algorithm runs left to right as a band: scene safe, call for help, breathing and pulse, start CPR, then the Shockable? diamond with one amber shock on the YES lane and the two-minute loop underneath. Its line turns down the right side through the issue date, credential ID and valid-until date into the QR code, with duration, assessment and level on the left and two signers at the foot.

About

Basic Life Support Certificate Template, Algorithm Portrait

Credentialing offices, staffing agencies and clinical placements often keep a portrait copy of each provider's life support card in the staff file, next to licences and immunisation records. A BLS certificate shows that the holder passed a provider-level course in CPR, AED use and team resuscitation on a given date, and the valid-until date tells a manager when the two-year renewal is due. Education departments, simulation centres and medical and nursing schools give it out at the end of the course.

Wording on the template“Passed the BLS skills test and written exam: high-quality CPR for adults, children and infants, early AED use, bag-mask ventilation and resuscitation as one team.”

Subject: name, your BLS certificate and badge are here

Hi name,

Well done on passing your Basic Life Support course. In the skills test you showed that you can run your part of a resuscitation: compressions, the AED, ventilation and clear teamwork.

Your certificate and badge are ready. Open them to check your details, download a copy for your staff file or credentialing office, print one if you need it, and share the badge on your profile. The valid-until date shows when your renewal is due.

Thank you for keeping your skills current for your patients and your team.

Post for the recipient to share

I completed my Basic Life Support renewal course this week. The skills test covered high-quality CPR for adults and infants, the AED, bag-mask ventilation and working as a team through a full scenario. Practising with colleagues always reminds me how much clear roles matter. Thanks to our instructors for a focused session. #BLS #Healthcare #Nursing

Printing

Printing it well

Print at actual size

Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.

Use card, not paper

A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.

Frame it

A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.

Mind the edges

Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.

Certificate or badge?

Send both. They do different jobs.
