Design notes
The portrait page gives the route more room. The name, the program and the statement fill the upper half over a four-column record row: duration, format, date issued and expiry date. Below, the route winds down a street grid in three rows past five visits, from personal care to reporting changes, and ends at the QR code between two signatures. A good choice for a printed record to file or frame.
About
Home Health Aide Certificate Template, Visit Route Portrait
A home health aide course prepares people to care for older, ill or disabled clients at home, under a care plan that a nurse sets. Federal rules for Medicare-certified home health agencies ask for at least 75 hours of training, with at least 16 of them supervised practical training. Many schools and agencies mark the end of that course with a certificate the new aide keeps in their employment file. It records the training; the provider issues it, not the state.
Wording on the template“Completed classroom and supervised practical training in caring for clients in their own homes: following the care plan, keeping each visit safe and reporting changes to the supervising nurse.”More wording ideas for home health aide certificates

Subject: name, your home health aide training certificate is ready
Hi name,
Congratulations on finishing the Home Health Aide Training Program. You put in the classroom hours, practised each skill under the supervising nurse and showed that you can care for clients safely in their own homes.
Your certificate of completion is ready at the link below. You can download it, print a copy for your employment file and share it with home care agencies when you apply.
We are proud of you and wish you well with your first home visits.
Hyperstack fills in each person’s name and link when it sends.
Post for the recipient to share
I have finished my home health aide training: classroom lessons and supervised practice in personal care, vital signs, home safety, infection control and reporting changes to the nurse. Thank you to my instructors and to the clients who welcomed us into their homes. I am ready to start my first home visits. #HomeHealthAide #HomeCare #Caregiving

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
