Design notes
This landscape layout is drawn as one line of that record: a register with columns for date, activity, provider and contact hours, two faint earlier rows, and the new entry marked in amber with a ledger tick, its hours set as a large serif figure over a double rule. Under it sit the format, the provider number, the credential ID, a short statement, the topics covered, a signature with its role and a QR code. The accreditation statement is a placeholder that the provider fills in with the exact wording its approving body gives. It suits hospital education teams and CE providers that want a calm, clinical record.
About
Nursing CE Certificate Template, Record Entry Landscape
Many state boards of nursing and certifying bodies ask nurses to complete a set number of continuing education contact hours in each renewal period. Hospital professional development departments, nursing associations, schools of nursing and independent education providers issue a certificate when a nurse finishes an activity and its evaluation. The nurse keeps it in a CE record and shows it if the board audits the renewal.
Wording on the template“Completed this nursing continuing education activity in full, including the learning assessment and the activity evaluation, and is awarded the contact hours recorded here.”More wording ideas for nursing continuing education certificates

Subject: name, your CE certificate and contact hours are ready
Hi name,
Thank you for completing the activity and its evaluation. Your continuing education certificate is ready, with the date, the contact hours you earned and our provider details.
Open it to view it, download a copy for your CE file or print it for your records. Keep it with your other certificates, in case your board asks for proof at renewal. You can also add the badge to your LinkedIn profile.
We hope the activity was useful in your practice.

Post for the recipient to share
Another line in my continuing education record: I completed Wound Care Essentials and earned 2.0 contact hours. Good refresher on staging pressure injuries and choosing the right dressing, and I am already using it on the unit. Thank you to our education team for making time for learning on busy shifts. #Nursing #ContinuingEducation

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it on its side, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
