Design notes
The tall page turns the record entry into a card. Under the centred name, the amber card shows the activity with its contact hours set large over a double total rule, then the date, format, provider, provider number and credential ID in labelled rows. Below come the statement, the topics covered in two columns, a signature with the signer's role and a QR code for verification. The bracketed accreditation statement is only a placeholder: the provider replaces it with its own approved wording. A good fit for staff development teams that file records on paper as well as online.
About
Nursing CE Certificate Template, Record Entry Portrait
A portrait page fits the binder or folder where a nurse keeps proof of learning next to a licence and specialty certifications. Each continuing education activity, such as wound care, medication safety or pain management, is counted in contact hours, and the certificate is the nurse's evidence that it was completed. Clinical educators, nurse planners and CE providers usually issue it once the learner has finished the content and the evaluation.
Wording on the template“Has completed this nursing continuing education activity in full, including the learning assessment and the activity evaluation, and is awarded the contact hours recorded here.”More wording ideas for nursing continuing education certificates

Subject: name, your CE certificate and contact hours are ready
Hi name,
Thank you for completing the activity and its evaluation. Your continuing education certificate is ready, with the date, the contact hours you earned and our provider details.
Open it to view it, download a copy for your CE file or print it for your records. Keep it with your other certificates, in case your board asks for proof at renewal. You can also add the badge to your LinkedIn profile.
We hope the activity was useful in your practice.
Hyperstack fills in each person’s name and link when it sends.
Post for the recipient to share
Another line in my continuing education record: I completed Wound Care Essentials and earned 2.0 contact hours. Good refresher on staging pressure injuries and choosing the right dressing, and I am already using it on the unit. Thank you to our education team for making time for learning on busy shifts. #Nursing #ContinuingEducation

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
