Design notes
This landscape design draws the Swiss cheese model taught in most safety courses. Five green slices stand in a row and an orange hazard arrow passes through the holes of the first four before the last slice stops it. Each slice carries one skill learned, so the drawing is also the list of competencies. The name, program and a short statement are set on the left. Two signatures, the issue and valid-until dates, the duration, and the level run along the base with the credential ID and a verification QR code. It suits induction programmes, site safety courses and annual refreshers.
About
Safety Training Certificate Template, Swiss Cheese Model
Employers, safety consultants and training providers give a workplace safety training certificate when an employee finishes an induction, a basic health and safety course or a refresher. A good course teaches how to spot hazards, assess and control risk, report incidents and near misses, follow safe systems of work and respond in an emergency. The certificate records that training for the worker's file and for audits.
Wording on the template“has completed training in workplace health and safety and has shown the competence to identify hazards, assess and control risks, report incidents and near misses, work to safe systems of work and act correctly in an emergency.”More wording ideas for workplace safety training certificates

Please note the valid-until date, so you can book your refresher in good time. Thank you for helping to keep everyone safe at work.

Post for the recipient to share
I have completed workplace health and safety training. We practised spotting hazards, assessing risks, reporting near misses and responding in an emergency. My main lesson: every person who knows what to look for is one more layer of defence between a hazard and an accident. Thank you to our trainers for the practical sessions. #WorkplaceSafety #HealthAndSafety #SafetyTraining

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it on its side, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
