Design notes
The portrait puts the drawing at the top: five tall green slices in the Swiss cheese model, numbered 01 to 05, with an orange hazard arrow that the final layer stops. A numbered Layers of Defence column beside it names each skill, from hazard identification to emergency procedures. Below come the worker's name, the program and a statement of competence, then a data grid with the issue date, valid-until date, credential ID, duration and level, two signatures and a QR code. It is a clear, practical choice for employers who keep printed training records.
About
Safety Training Certificate, Swiss Cheese Model Portrait
A health and safety training record often goes into a personnel file, a site induction folder or a contractor pack, and a portrait page fits those binders and printers best. Companies issue it after basic safety training, a new starter induction or a periodic refresher, and many set a valid-until date so a supervisor can see when the next course is due.
Wording on the template“has completed training in workplace health and safety and has shown the competence to identify hazards, assess and control risks, report incidents and near misses, work to safe systems of work and act correctly in an emergency.”More wording ideas for workplace safety training certificates

Subject: name, your workplace safety training certificate is ready
Hi name,
Well done on completing your workplace health and safety training. Your certificate is ready.
It records the skills you practised, from spotting hazards and assessing risks to reporting incidents and knowing what to do in an emergency. You can download it, print a copy for your training file, or share it on LinkedIn. Anyone can confirm it is genuine with the QR code.
Please note the valid-until date, so you can book your refresher in good time. Thank you for helping to keep everyone safe at work.
Hyperstack fills in each person’s name and link when it sends.
Post for the recipient to share
I have completed workplace health and safety training. We practised spotting hazards, assessing risks, reporting near misses and responding in an emergency. My main lesson: every person who knows what to look for is one more layer of defence between a hazard and an accident. Thank you to our trainers for the practical sessions. #WorkplaceSafety #HealthAndSafety #SafetyTraining

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
